We remind our patients that insurance is considered a method of reimbursement, and not a substitute for payment. Services in our office may be covered by dental insurance, medical insurance, or a combination of both. Certain emergency situations may involve the use of auto, workers compensation or other accident insurance.
Please bring all pertinent information to your initial visit. Many patients have insurance that covers a portion of their treatment.
We recommend (and the insurance carrier may require) preauthorization as a method of determining insurance benefits and patient obligation in advance of treatment.
A growing portion of our patients have coverage under a managed care plan (HMO, PPO, etc.). Please be familiar with the provisions of your coverage. Unless Dr. Sultan is a provider under your plan, be aware that insurance is a contract between you and your carrier. In cases where our office is a provider for your plan, you may be required to obtain authorization prior to your visit. You will be responsible for any deductibles, co-pays or non-covered services prior to treatment.
Although we are a Medicare provider, this program does not provide coverage for dentally related services.
If you have private insurance, or if we are not providers under your plan, you will be responsible for your treatment fees, and an insurance claim will be filed on your behalf.
We do have a number of healthcare financing options available. These programs will be discussed in detail at the time of your consultation and prior to any treatment rendered. Please allow our staff to advise you so we can facilitate your visit.

Please be aware that additional facility and anesthesia services may be involved in your care. Separate charges may be applicable and due that are separate from surgical fees. These will be explained in detail by our financial advisor.
