Orthognathic Surgery Recovery Timeline: Week by Week

Woman opens her mouth with red lipstick to do jaw exercises after surgery. Orthognathic surgery is a transformative procedure that corrects jaw misalignment and improves both appearance and function. Because of its complexity, you may have questions about what recovery will look like — and how long it will take to return to normal.

At Sultan Center for Oral Facial Surgery, Dr. Leslie Sultan brings over 30 years of surgical expertise to complex jaw procedures and provides each patient with a carefully structured recovery plan for the best outcomes.

Below is a general recovery timeline that outlines what most people can expect in the weeks and months after orthognathic surgery.

Week 1: Managing Swelling and Discomfort

The first few days after surgery are typically spent resting. Swelling peaks around days 2–3 and may involve yourcheeks, lips, and even under your eyes. Bruising is common. A liquid diet is required, and our team will provide pain management, antibiotics, and detailed post-op instructions. During this time:

  • Use cold compresses to help reduce swelling
  • Sleep with the head elevated
  • Maintain oral hygiene with gentle rinsing

While your face may feel numb or tight, these sensations are normal and gradually improve.

Weeks 2–3: Early Healing and Light Activity

Swelling begins to subside, and you should start to feel stronger. You may return to light daily activities or work (depending on the physical demands), but any heavy lifting or exercise should still be avoided. Diet remains soft or blended, and any elastics or splints placed during surgery are monitored closely. Expect:

  • Improved energy levels
  • Slight return of facial sensation
  • Beginning to adjust to your new jaw position

Weeks 4–6: Regaining Function and Jaw Mobility

By this stage, swelling is significantly reduced, and early bone healing is underway. Dr. Sultan may adjust splints or elastics depending on your case. You can usually begin light jaw exercises under supervision, but continue to eat soft foods. At this point:

  • You may notice improved bite alignment
  • Jaw movement becomes easier
  • Post-op appointments monitor healing progress

Weeks 6–12: Transitioning to Normal Diet and Activity

By now, you are generally cleared to resume normal daily activity, including moderate exercise. Depending on your surgical plan, you may begin introducing firmer foods or resume more active physical therapy. Ongoing healing continues beneath the surface, even though you may feel nearly back to normal.

3 Months and Beyond: Long-Term Stability and Results

Full bone healing takes several months. While most of the visible healing occurs in the first 6 weeks, your jaw will continue to stabilize for up to 9–12 months. Post-operative follow-ups ensure your bite, airway, and jaw function remain ideal. Any remaining numbness or stiffness usually resolves over time.
